Filling Health Forms

This  is one  of  the  basic  requirements  of  your elective application. All  of  us  already have  been  immunized against the diseases recommended by WHO  in  the  childhood, it is always advisable to keep their documented proof/certificate with you. To see for which diseases you need to be vaccinated(I have made a general list of most commonly required immunizations), visit the "Immunization Certificate".
 

  • If you have not being vaccinated against these diseases or dont have a documented proof of your vaccinations, it is strongly advised to get yourself vaccinated and then get a certification that you have been vaccinated against the mentioned diseases and keep the certificate with you for future reference. Some of the universities have their requirements of few additional vaccines. It is strongly advised to go through the health forms before being vaccinated to prepare a list of all the vaccinations you need to have and then get yourself vaccinated for all of them.
  • For all the Universities you need to complete your health forms and provide proof of all the required immunizations i.e titers after being vaccinated.
  • After getting these titers done, get a chest X- Ray done and your Mantoux Test done. Dont forget to save the reports/records of all these (vaccinations, titers report, X-Ray report, Mantoux test report) with you.
  • Now with all the above mentioned documents, go to any doctor(I mean any...from GP till Professor, any certified doctor i.e RMP in Pakistan) and request him to fill your forms according to the reports/records provided by you and ask him to sign your health form. You can also fill the form for him and then request him to counter-check everything from the provided documents before signing.

sir,

1.is it necessary to get the health forms of different universities stamped by the same lab wherefrom titres are procured?or titres from any lab+any health care provider will suffice?

 

2.i recieved vaccination for hep n varicella in 2008,but have no dcoumented proof:(or bill...as it was an over the coounter thing..so titre evaluation is the only way out?my titres came negative also:(

 1. no I dont think so...I did not do so...

2. well I believe titers can help in few places but keep in mind...for most places..there is no substitute of documented evidence of vaccinations... tc and best of luck!

Hello Umar Bhai,

I intend on doing electives at Mount Sinai...However there is one little problem...Not a single lab(not even Aga Khan Hospital) here in karachi does IgG titer for measles...and Mount Sinai has stated that positive Measles titer serology  required from foreign students...Now i have a vaccination certificate that says i got vaccinated for measles...u think theyll accept my certificate? Kindly reply

It is better if you have titers...

Try contacting some of the other labs in Karachi... consult some of your seniors at AKU... m sure someone might be able to help u...

I know a few labs in Lahore that might be helpful...

                   Like many, I too have been a silent visitor of your website for the past couple of months. What a wealth of information I must say it is. Indeed a blessing for all the confused minds.

      My question here is that you mentioned there are a few labs here in Lahore that do the serology tests i.e. titres, so could you please mention their names and locations as having already been refused by AKUH and Shaukat Khanum labs I'm having a hard time finding any such lab for MMR/chicken pox titres. I dont remember their names but there were quite a few in Shadman; moreover trying contact the pathology department at your medical college; they can guide you the best...

I've got same issue. I got titers for Hepatitis B, rubella and Varicella too from Agha Khan. They said that they can't provide titers for measles (3 months back) but last night I checked their laboratory website and there was a test available for Measles antibodies. I'll ask them again about it. About mumps i'm still finding a lab which provide titers for it in reports. Anybody else with some helpful tips to share plz help.

what is the document that will prove i got vaccination today ,  is it the doctors prescription or just his sign and assurance that  this boy has reccieved vaccine ,,,  or again titres? .... 


i dont have any proof for vaccinations i had , is there any chance of titres being good or high ,,, or   i better dont waste time , and get vaccinated. after vvaccintion , should i get titres ,, and when   ?  after some days .

For all those worrying abt titers

    There is no such lab in pakistan according to my knowledge in pakistan which does the MMR and varicella(chicken pox) titres. I know this becoz in our times only one of  our friend (who was in Saudia)got the original titers and for that he had to send his blood sample to USA, which wud cost huge money So if Agha khan and shaukat khanum are not doing it, then no lab in Pakstn can do it. The best way is to avoid the university which is asking for titers. I knw university of connecticut ask for titers n they usually dnt give much attention to international applicants. So just get ur immunization certificate and health form signed. Remember Immunization certificate is enough for majority of universities.

Is live attenuated Hep A vaccine better than Inactivated?

Somebody kindly help me regarding this.

Wockhardt company manufactures live attenuated. Dose 0.5ml for both adults and children. Single dose is sufficient. No booster dose required.Laboratories suggest that there are better titres with live. Is the efficacy proven?

Havrix by Glaxo SmithKline, inactivated veccine- 1ml, 2doses required.

1.Is there any protocol for this vaccine as to whether it has to be live or inactivated while applying with immunization certificates?

2. Which vaccine is better? Live or inactivated?

I got HbV vaccine but i didn't get certification. If I signed a certification from any doctor or any hospital that I got any vaccine ,is this a proof to avoid titration ?

hi, i need to get done adult TDaP booster for my elective. But Aga Khan and LNH doesent have it. Then i came to know its only TD for adults. Do you have any idea from where i can get this?
